Labs & Imaging
Want to use your insurance? We can send your lab order to any of the local hospital systems. We also offer reduced cash prices for many labs and imaging. Click here for an example pricing sheet. We have various options for imaging pricing, but here are cash prices for one imaging location.
We can also draw your labs in-house, so you don’t have to go elsewhere!

Medications
You have options. Use insurance or pay cash. Get your prescriptions through mail order, local pharmacy, or our in-house pharmacy. We carry some common medications in the office and can order many more. They usually arrive the next day, and we will ship them to your house upon request! Click here for sample pricing. If you don’t see your medications listed there and want to know the pricing, email us a list, and we will let you know!
Advanced Testing
Your membership includes exceptional primary care — but some in-depth lab tests require additional time, careful interpretation, and a more personalized approach.
To ensure you receive the attention these results deserve, these services are offered as add-ons outside of routine care. Please inquire for details about test options and associated fees.
Billing & Payment Policies
Each new billing account will charge a one-time, non-refundable enrollment fee.
NO LONG-TERM CONTRACTS or commitments; however, a re-enrollment fee (two times the initial fee) will apply if you decide to rejoin at a later date.
Membership fee(s) are started upon enrollment. Membership fees are paid by the calendar month on an ongoing basis until cancellation. There is a 30-day cancellation policy. Payment of membership fees is required to remain an active member and receive service.
ALL members must enroll for Auto-Pay of invoices with a Checking Account, Credit card, or pin-less Debit Card. Due to our low staff and overhead business model, we cannot invoice or bill membership or other fees. The auto-pay date may be selected for any day of the month during set-up.
The maximum monthly membership fee per nuclear family (parents with up to 3 dependent children under 22) is $200. Additional dependents may be added for $10 each.
